Page 1 of I Docket No. 50-339 Report No. LER 80-015/03L-0 Description of Event On May 25, 1980, during Mode 3 operation, maintenance personnel were attempting to enter the containment through the personnel a. cess hatch. The hatch outer door failed to close due to a loss of hydraulic pressure in the closing system.

Probable Consequences of Occurrence The consequences of this event were limited since the unit was in Mode 3 prior to initial criticality and due to the presence of an inter-lock p.cventing the opening of the inner door while the outer door is opened. Therefore, the health and safety of the public were not affected.

Cause of Event The loss of hydraulic pressure was due to the failure of a connection to the hydraulic system pressure gage. This prevented the system from developing sufficient pressure to close the door.

Immediate Corrective Action A maintenance request was submitted. The system was repaired, tested and placed in service.

Scheduled Corrective Action No further action required.

Actions Taken to Prevent Recurrence No further action required.
