APPENDIX A Alaska Welding Center Radiography Department 3021 Davis Road Fairbanks, Alaska 99701 License No. 50-19202-01 l'0TICE OF VIOLATION Based upon the results of the NRC inspection conducted on September 18, 1980, two activities under your license appeared to be in noncompliance with NRC requirements as identified below:
1.
NRC Regulation 10 CFR 20.408(b) reauires that when an individual terminates employment with a licensee, the licensee shall furnish to tne Director of Management anc Program Analysis, US NRC a report of the individuals exposures to radiation incurred during the period of emoloyment by the licensee.
Contrary to the above requirement reports of exposures to radiation incurred by four individuals terminated during the internal May 23, 1980 to July 18, 1980 were not sent to the above address.
This is a deficiency.
2.
flRC Regulation 10 CFR 19.13(d) requires that when an employee terminates employment with a licensee that the licensee shall provide the former employee a report of his exposures received while employed by the licensee.
Contrary to the above requirements, four individuals terminated during the interval "ay 23, 1980 to July 18, 1930 were not sent reports of exposures to radiation received while employed by the licensee.
This is a deficiency.
