Farley Unit 1 Report Date: 2/29/80 Event Date: 2/1/80 Identification of Event The "A" Train Component Cooling Water Loop failed to meet the requirements of Tech. Spec. 3.7.3.1 which states in part that two trains of Component Cooling Water will be operable.

Description of Event At 0400 on 2/1/80 the IB Component Cooling Water (CCW) Pump ("A" Train) failed to auto-start during the performance of "A" Train Loss of Off-Site Power Test. The pump could still be manually started from the Main Control Board.

Designation of Apparent Cause The IC CCW pump which is the normal "A" Train pump was removed from service on 10/5/78 for repairs and the 1B CCW pump (swing pump) was aligned to the "A" Train. On 2/1/80 the IB CCW pump failed to auto-start during the "A" Train LOSP Test. Upon investigation it was discovered that at the time of the test the IC CCW pump handswitch position (Auto Af ter Start) prevented an auto-start of the IB CCW pump. To ensure auto-start of the IB pump, the IC pump must be in

" Auto Af ter Stop" and the IB pump in " Auto Af ter Start". Presently there is no way to visually determine the position of the handswitches (i.e., " Auto After Stop" vs " Auto After Start" since the switches are three position (Stop-Auto-Start) and spring return to Auto.

Analysis of Event The IC CCW pump handswitch was positioned in " Auto Af ter Start" on 5/4/79 during the performance of FNP-1-ETP-113 " Train A Sequencer Breaker Closure Verification". This handswitch was not repositioned after the test, since it was not known that the IC handswitch being in " Auto After Start" would adversely affect the auto-start feature of the IB pump.

Corrective Action In troubleshooting it was discovered that the logic requires the 1C pump handswitch to be in " Auto Af ter Stop" position for the IB pump to auto-start. The IC handswitch was placed in " Auto After Stop" and the test successfully completed.

80-012/03L-0 In accordance with a minor departure f rom design evaluation, a jumper has been installed to negate the effect of the 1C hand-switch position on the 1B pump ("A" Train) auto-start. The test was performed again and successfully completed. The jumper will be removed prior to 1C CCW pump being returned ta service.

A design evaluation will be made to evaluate the auto-start logic of these pumps.

Appropriate personnel will be reinstructed on the auto-start logic for CCW pumps.
