y,TTACllMENT TO LER 78-106/03L-0 Page 1 of 1 During normal operation, plant Operations personnel noted that the coupling key for containment pressure suppression pool (torus) nitrogen purge supply valve (27-A0V-115) was disengaged. This effectively disconnected the valve disc from the valve operator thus making the valve inoperabic which is contrary to Technical Specification, Appendix A, Paragraph 3.7.D.1.

h' hen the coupling key is disengaged the valve disc (butterfly) is not restrained by the valve operator and thus the position might change from its nornal shut position to some intermediate or open position in the event the torus was pressurized. If the other isolation valve in the same line was inoperable and open at tne same time, the line would be open to the containment atmosphere dilution (CAD) system.

The cause of the coupling key being disengaged is unknown. The key was returned to its engaged position and clamped in place. Similar valves and coupling are utilized on the other containment isolation valves associated with drywell and torus vent, purge and inerting lines. Each of these valves was checked to assure that the coupling key is properly engaged.

In addition, Maintenance personnel noted that it may be possible to install a set screw in the coupling device to assure the key can not disengage. This possible additional corrective action is being discussed with the coupling vendor and will be reported on a follow-up report, if such action is taken.
