NAME OF PREPARER PHCNE:

ATTACHMENT TO LER 78-043/01X-1 Page 1 of 1 Revision 0 of this LER was submitted on June 23, 1978 and was the 14-day follow-up of a prompt report made by telecopy on June 16, 1978. The prompt report and follow-up provided notification and details relating to the environmental qualification of motors associated with the following motor operated valves:

20-50V-8 2 - Drywell floor drain sump inboard isolation 20-50V Equipment drain sump inboard isolation 13-MOV RCIC steam supply inboard isolation In each case the motor was supglied with electrical winding insulation qualified to 250 F rather than 295 F which could be reached during a design basis loss of coolant accident.

During the 1978 refueling and maintenance outage, each of these motors was rewound with Class H insulation (which meets the 295 F requirements). Following operability testing of the valves, the components were declared fully operable and returned to service without restriction.
