TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-79-157 DATE OF EVENT: December 29, 1979 FACILITY: Davis-Besse Unit 1 Failure of the IBM System 7 Security / Fire / Radiation IDENTIFICATION OF OCCURRENCE:

Computer The unit was in Mode 5, with Power (MWT) = 0, and Conditions Prior to Occurrence:

Load (Gross MWE) = 0.

Description of Occurrence: On December 29, 1979 at 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br />,5an operator noticed that the Contact Data Logger in the control room had not print out an alarm in over a day. This system presently logs fire and radiation monitor alarms To check(the thedoor sys-alarms have already been transferred to the new computer system).

tem, the operator tested a radiation alarm to see if the Contact Logger System would acknowledge; it did not.

The IBM System 7 Contact Data Logger was declared inoperable. The unit entered the action statenent of Technical Specification 3.3.3.8 the accessible which requires a fire patrol be established within an hour to inspect zones at least once per hour.

A check of the printout of the local Designation of Apparent Cause of Occurrence:the system became locked into an invalid door typer in the computer room showed thatWithin the IBM System 7, the memory which alarm. This indica for the door is used, indicated but did not clear when the message was printed. to be caused the buf fer that holds the messages to overflow and the words " msg lost" printed. Due to this condition, the Systen 7 would not allow any other alarms to be acknowledged, and therefore, nothing further was typed.

Analysis of Occurrence:

There was no danger to the health and safety of the public or to station personnel. An hourly fire patrol was established.

Corrective Action: On December 31, 1979, at approximately 1735 hours0.0201 days <br />0.482 hours <br />0.00287 weeks <br />6.601675e-4 months <br />, the error The system was placed back in was cleared, and the computer program was reloaded.from the action statement of Technical operation thus removing the unit 3.3.3.8. Work is continuing on phasing out the System 7 Contact Data Logger per Facility Change Request 78-083.

Failure Data: There have been numerous failures of the System 7 Contact Data Logger, see Licensee Event Reports NP-33-79-71, NP-33 58, NP-33-79-46, and NP-33-79-30.
