RESPONSE TO NRC BULLETIN 88-05 SALEM GENERATING STATION UNIT NOS. 1 AND 2 HOPE CREEK GENERATING STATION DOCKET NOS. 50-272, 50-311 AND 50-354 The purpose of this bulletin was to request licensees to submit information regarding flanges and fittings supplied by PSI and WJM to assure that the flanges and fittings comply with ASME Code and design requirements or are suitable for their intended service or that the flanges and fittings otherwise be replaced.

An industry-wide effort, overseen by NUMARC, was undertaken to generate a comprehensive database of test results from which conclusions could be reached regarding the extent of the problem and the validity and consistency of indicated material specifications. Based on the test and analytical results available as of August 3, 1988, NRC permitted (in Supplement 2 of the Bulletin) the temporary suspension of the field measurements, testing, records review, and preparation of justifications for continued operation (JCOs) that were requested by Bulletin 88-05*

and Bulletin 88-05, Supplement 1.

The written report requested in Reporting Requirement 1.b of NRC Bulletin 88-05 (as _amended by Supplement 1) is presented below.

Provide the information indicated in items 2a and 3a (of the bulletin) that indicates which flanges and fittings have been found not to be in conformance with the applicable code requirements or procurement specifications, confirms completion of other actions requested in items 2b or c, 3b and 4, and provides a schedule for any remaining actions.

RESPONSE

The following WJM material was found not to be in conformance with the applicable code requirements based upon results obtained by in-situ testing utilizing the Equotip Hardness Tester, the testing method endorsed by the NRC to be the industry-wide test method for compliance with Bulletin 88-05. *

FLANGE 1.00 11 150# WJM TO PSE&G Of the twenty-three (23) non-conformances reported, twenty (20) were found to be slightly (approximately 6% on the average) below the minimum specified Brinnell Hardness value of 137 HB, and the remaining three (3) were slightly (approximately 4% on the average) above the maximum specified hardness value of 187 HB.

Nineteen (19) of the 23 flanges were found to be installed in Salem Unit 1. The other four were installed in Salem Unit 2. No nonconf ormances were noted against the Hope Creek Generating Station.

Justifications for continued operation (JCOs) were being prepared for the above noted nonconformances when NRC Bulletin 88-05, Supplement 2 was received on August 3, 1988 granting the temporary suspension of the response effort. However, the in-situ test results noted above did not reveal hardness deviations beyond a tolerance of +4% and -6% of specification range (137-187 HB) and the preliminary analysis indicated ample design margin to support continued operation with no significant safety concern. Also, JCOs were generated for thirty-two (32) flanges which were determined to be inaccessible for in-situ testing.

In total, the review of procurement documents for direct and indirect purchases of WJM and PSI flanges and fittings resulted in the identification of 2692 flanges/fittings as being received by PSE&G. This quantity represents the results of our review in response to the actions requested by NRC Bulletin 88-05. This review was considered to be approximately 75% complete as of August 3, 1988.

Documentation of the specific actions taken in response to the Action Items of NRC Bulletin 88-05 and its supplements for identified materials has been maintained for inspection purposes.

In addition, representative samples of WJM/PSI items subjected to test by our independent approval laboratories have been retained.
