/:00 am 8:00 am 8:05 am 8:10 am NUCLEAR ACCIDENT SCENARIO PALISADES EXERCISE February 23, 1982 Sequence of Events Initial notification is provided by the Palisades Nuclear Plant that an "Unusual Event" has occurred due to high specific activity in the primary coolant system.

This notification is provided to the State Police at the South Haven Post and to the Van Buren County Sheriff's Dispatch.

The State Police forwards the information on to Lansing Operations in accord-ance with established procedures.

Operations contacts Emergency Services Division Duty Officer and Radiological Health, Department of Public Health, and indicates the situation is under control.

The Palisades Plant Shift Supervisor is investigating reason for the increase and has indicated that Technical Specifications have been exceeded and an orderly shutdown is in progress.

ESD/MSP and RHD/DPH concur via telephone that this "Unusual Event" does not warrant a Governor's Disaster Proclamation or activation of the Michigan Emergency Preparedness Plan.

While the state assessment occurs, local officials in Van Buren County have received their initial notification through Van Buren County Sheriff's Dis-patch.

Allegan and Berrien counties are also notified.

Due to the classifi-cation, no mobilization is necessary.

Van Buren County is normally prepared to provide routine security, fire fighting, emergency medical assistance, etc., but no requests are expected from the plant.

The Palisades Shift Supervisor has difficulty determining the source of the increased radioactivity within the primary coolant system.

As a result, he decides to begin shift augmentation though he is not required to do so.

The objective of this action is sim~ly to provide an opportunity to test the plant's ability and to also assist in deter.nining the cause of the current problem.

Specific activity within the primary coolant system continues to rise alarmingly and demands an escalation in the classification to "Alert".

A follow-up message from Palisades of a nuclear incident that has been re-classified to an "Alert" is received by MSP South Haven.

The same message is received at the Van Buren County Sheriff's Dispatch.

MSP South Haven forwards "Post Disaster Report" and follow-up message to Lansing Operations.

Operations notifies ESD/MS? and RHD/DPH and they confer.

ESD/MSP notifies the Governor's Office and evaluation continues.

Within the same time frame, Van Buren County Dispatch utilizes its call list and appropriate officials are informed.

The counties of Allegan and Berrien are also provided information concerning the "Alert" classification and begin their mobilization.

Based on the potential threat, the Van Buren County Board of Commissioners Chairperson declares a "State of Emergency" resulting in an order to mobil-ize county personnel at the Emergency Operating Center.

Allegan and Berrien counties follow the Van Buren County declaration and declare for their respective counties.

Full mobilization of all necessary EOC staff continues for approximately the next 30 minutes.

Plant Status Update Message #lreceived by MSP South Haven and Van Buren Sheriff's Dispatch and forwarded as appropriate.

Based on the current analysis of the plant conditions, the Governor declares a "State of Disaster" pursuant to Act 390, P.A. of 1976.

Upon this declara-tion, the ESD/MSP notifies the State Department of Agriculture, Commerce, Military Affairs, Natural Resources, Public Health, Social Services, State Police, and Transportation to dispatch departmental emergency services coordinators to the State EOC (Lansing), and to notify appropriate field personnel to prepare to report to Saugatuck State Police Post to staff the On-scene State* EOC, when it becomes operational.

The SEOC in Lansing is fully operational and open telephone lines are estab-lished with the county EOC's, the plant, and Consumer Power's Corporate Operations Center in Jackson.

Assessments continue and no protective action guides are issued.

Exercise action commences.

Plant Status Update Message #2 received and forwarded accordingly.

The plant also provides meteorological data with the following weather conditions noted:

the wind is out of the W.S.W. at approximately 10.5 mph at the lake front; temperatures are in the teens with an expected high of 21° ?ahrenheit.

Based on these conditions, a Stability Class "E" is projected for any release.

Further, light to medium snow flurries are expected throughout the area with accumulations of an inch or more, and some drifting expected.

Upon notification by ESD/MS?, Operations Division dispatches com.~and trailer

  1. 1 to predesignated location of the OSEOC at Saugatuck State Police Post.

E.T.A. is 11:30 am based on current road conditions.

Also, RHD/DPH has dispatched its Radiological Response Teams to the scene and their E.T.A.

at Saugatuck is 11:00 am.

Until the Governor declares the couuties will receive recommendations from the plant and verify same through the Lansing EOC.

State analysis continues from Lansing with projection of off-site effects being developed.

Plant Status Update Message #3 received and forwarded accordingly.

The Governor's Press Section issues a news release to the AP and UPI wires that the Governor has declared a "State of Disaster" as a result of an acci-dent at the Palisades Nuclear Plant near South HaYen, Michigan.

The release stated that due to internal problems within the plant, high levels of radiatio:

within the primary coolant system have been detected.

The plant has declared an "Alert" and notified the NRC, the State, and the local governmental units.

At this time, no protective actions are warranted, but state and local emer-gency response personnel are being mobilized and are on standby status.

Time 9:00 am 9:01 am 9:15 am 11:30 am (State Simula-tion) 1:00 pm Exercise Controllers Internal Plant Conditions: a turbine trip occurs and the main steam isolation valves close.

As a result, atmospheric steam dump valves open to release pressure.

After the pressure lowers significantly, the atmospheric steam dump valves close again but one fails to seat properly.

Reactor trip.

For undetermined reasons, several steam generator tubes rupture.

Since one atmospheric steam dump valve has not seated properly, radioactivity is released into the-atmosphere.

A "General Emergency" is declared immediately.

The release concentrations are significant enough for a recow.mendation by the plant to state authorities for a minimum 3-mile evacuation.

(Dose and dose rates projected by the State from plant source term data are attached).

A simulated evacuation will take place for all plant non-essential personnel due to the high radiation levels.

This radiation release will continue until 11:15 am.

At approximately 11:30 am, the On-scene State EOC in Saugatuck will simulate its operation.

At this point in the exercise, the secnario clock will be advanced to accommodate an orderly transition for all agencies into the recovery/re-entry phase of the exercise.

A simulation of the Governor's proclamation authorizing re-entry will be made including sa..~pling results obtained by the State ~ealth Department.

Officially, the exercise will end at 1:00 PM and will be followed by a criti~ue at the local levels.
