ENCLOSURE TURKEY POINT STATUS REPORT (OCTOBER 8-30, 1987) 1.
Plant Status Units 3 and 4 are currently in Mode 5, conducting minor maintenance.
2.
Facilit 0 erations Summar Unit 3 remains down (from last report) due to replacement of electrical leads to an RHR pump and the replacement of a purge exhaust valve.
The Unit is presently scheduled for restart on November 11.
Unit 4 was shutdown on October.
12, 1987 due to Hurricane Floyd.
It has remained shutdown since then due to a Safety Injection pump that was out of service and repair of several minor leaking valves.
On October 21 and several times on October 25,
- 1987, a
leaking accumulator isolation valve resulted in a reactor vessel head void due to Nitrogen intrusion.
The accumulator, vessel and pressurizer were vented.
During venting of the accumulator, a
normally open valve for accumulator venting was found closed.
The plant is investigating the valve closure.
On October 28 another Nitrogen bubble was detected in the vessel
Restart is scheduled for November 10.

Item(s) of S ecial Interest On October 19,
- 1987, FPL was issued a Civil Penalty in the amount of
$225,000 for failure to assure configuration control of emergency systems (emergency boration
Additionally, an order was issued confirming FPL s initiative to conduct an independent management review and to continue on-shift operations monitoring.
The concept of an independent management review of the Turkey Point facility and the FPL corporate organization was initiated by FPL several months ago.
Additional details of the review were discussed with the NRC in a
management meeting in Atlanta on September 25, 1987.
Due to the violations that led to the Civil Penalty and the manipulation of controls by an unlicensed
,individual, this third party management review was confirmed in an NRC Order.
In August 1987, FPL initiated a Management-on-shift program in an attempt to identify and correct operations problems that seemed to occur primarily on the back shift.
It was through this program that the unauthorized reactivity manipulation was identified.
FPL expanded this program to cover all shifts and to use non-Turkey Point employed operationally qualified evaluators.
This program was also confirmed in the NRC Order.
The Licensee's expanded on-shift program was implemented on October 23, 1987 with the issuance of Administrative Procedure O-ADM-019, Management-on-shift (MOS).
Currently, four FPL and two Westinghouse SRO-qualified individuals are providing round-the-clock, on shift coverage.

NRC Staff Activities Durin the Period On October 26, the NRC terminated our augmented inspection coverage.
This involved continuous on-site coverage from September 25 to October 9 and a
reduced coverage (approximately 50 percent on backshifts and weekends) from October 9 to October 26.
Fourteen NRC personnel including the three Turkey Point Resident Inspectors, both St. Lucie Resident Inspectors, a
Resident Inspector from Surry, the NRR Senior Project
- Manager, four Operator License Examiners and three Regional-based inspectors partici-pated in the on-shift coverage program.
Details of their findings will be contained in Inspection Report 87-44.
On October 28, An Enforcement Conference was held in the Regional Office concerning previous violations in the area of Security.
