Revision 15 (03/07)

FIRE PROTECTION SYSTEM FAILURE MODE AND EFFECTS ANALYSIS Component Failure Mode Probable Cause Effect on System Inherent Self-Regulating Measures Control Room Indication Remarks

(DRN 06-874, R15)

1) Jockey Pump Failure to start or stops running Loss of power, or mechanical malfunction, or controller trouble.

Fire main pressure continues to drop Motor driven fire pump starts

1. Jockey Pump Trouble
2. Motor driven fire pump running.

Jockey pump is used to maintain fire main pressure and its loss does not affect the systems firefighting capability.

(DRN 06-874, R15)

2) Motor driven fire pump Failure to start or stops running Loss of power, or mechanical malfunction, or controller trouble.

Loss of pumping capability and fire main pressure continues to drop.

First diesel pump automatically starts on drop in fire main pressure.

1. First diesel pump running.
2. Loss of power to motor driven fire pump.

1st diesel driven fire pump will provide adequate capacity to yard.

3. Motor driven fire pump "trouble."
3) First diesel driven fire pump Failure to start or stops running Loss of battery starting capa-bility and/or engine trouble.

Loss of pumping capability and fire main pressure continues to drop.

Second diesel pump automatically starts on further drop in fire main pressure.

1. Second diesel fire pump running.
2. First diesel fire pump "trouble."

2nd diesel driven pump is a standby fire pump which will provide adequate capacity to the yard fire loop. If this pump is activated, the system is considered to have an active single failure.

(DRN 06-874, R15) 4a) Underground yard loop Pipe rupture (under fire condition)

Deteriorated pipe condition Loss of water supply of fire Motor and possibly diesel driven fire pumps will start.

1. Jockey Pump Running.
2. Area fire alarms.
3. Motor and/or diesel driven fire pump running alarms.

Post indicating valves will be used to isolate damaged poriton of the loop. Valving is arranged to ensure a continued water supply to all remaining sections of the loop.

4b) Underground loop Pipe rupture yard (no fire condition)

Deteriorated pipe condition Water starts flowing at break.

Motor and possibly diesel driven fire pumps will start.

1. Jockey Pump Running.
2. Motor and/or diesel driven pump running alarms.

Post indicating valves will be used to isolate damaged portion of the loop. Valving is arranged to ensure a continued water supply to all remaining section of the loop.

Revision 11 (05/01)

FIRE PROTECTION SYSTEM FAILURE MODE AND EFFECTS ANALYSIS Component Failure Mode Probable Cause Effect on System Inherent Self-Regulating Measures Control Room Indication Remarks

5) FPS stand-pipe Pipe rupture (riser)

Deteriorated pipe condition or accidental damage.

Loss of hose con-nections supplied by riser.

Motor and possibly diesel driven fire pumps will start.

1. Motor and diesel engine driven fire pump running alarms.

Interior OS&Y control valves at base of riser provide isolation of that riser and discontinuation of flow.

(DRN 99-1018)

6) FPS auto-matic fire suppression systems Pipe rupture (no fire condition)

Deteriorated pipe condition or accidental damage.

Loss of fire sup-pression system for protection of area, equipment or components involved.

Motor and possibly diesel driven fire pumps will start.

1. Motor and/or diesel driven fire pump running alarms.
2. Alarm check valve flow alarms (wet pipe systems) or deluge control valve operating alarms (pre-action, and water spray systems).

Manual shutoff of water supply at system main OS&Y valves to facilitate repair.
