- 1. The reason for your access authorization denial is given in Section I of this form. If you want to request a review of this decision, you must complete this form.
- 3. You should be notified in writing of the disposition of your review within 30 days following receipt by NMC of all information supporting your review request. Pending the disposition of your review request, your access authorization will remain denied

Individuals are denied access where background information is developed adversely reflecting on trustworthiness and reliability.
I.e CASE FACTS
- a. On December 9, 2001 you acknowledged to an NMC investigator that while working at Kewaunee you smelled alcohol on a worker and did not report the observation to either Kewaunee supervision or security. When asked why you did not report the smell of alcohol, you stated that it was not your job to "sniff' the worker.

In regards to the statement in your letter, I believe there has been a misunderstanding or misinterpretation of what was discussed during our phone conversation.
This incident took place in the security access gate, not at my work station.
Furthermore, the individual or individuals who smelled of alcohol, would have been detained at the gate for "unfit for duty" reasons.
Later that morning, I was 'Called for a drug screen. After I completed my testing and returned to my work area, I confronted my foreman about the possibility that I was suspected of being drunk. He assured me that there wasn't any concern and that he would look into it. I also told him what happened earlier in the gate house. The maintenance shop supervisor also knew of the incident.
In conclusion let me say this, I have worked in nuclear facilities for six years and my record should speak for itself I do"my job in a professional manner and I am also a trustworthy person. I would not put myself or my fellow workers in such an unprofessional situation.
