Perry Nuclear Power Plant Docket No. 50-440, License No. NPF-58 NRC Post-Examination Documentation, Perry License Class 1 LOT21 10 CFR 55 In accordance with the guidance provided in NUREG-1021, "Operator Licensing Examination Standards for Power Reactors" (Revision 11 ), ES-501, "Initial Post-Examination Activities" Section C.1, the following materials are submitted in support of the Perry Nuclear Power Plant written operator license examination conducted on the weeks of February 19, 2021 :
- 1. Graded written examinations (i.e., each applicant's original answer and examination cover sheets) plus two clean copies of each applicant's answer sheet (ES-403, "Grading Initial Site-Specific Written Examinations")
- 2. Master written examination answer keys, (No changes made while administering and grading the examinations) (ES-402, "Administering Initial Written Examinations," and ES-403)
- 3. Questions asked by the applicants and answers given to the applicants during the written examination (ES-402)
- 4. Post-examination comments made by the applicants after the written and operating examinations. No questions were challenged by the applicants.
- 5. Seating chart for the written examination (ES-402)
- 6. Completed Form ES-403-1, "Written Examination Grading Quality Checklist" (ES-403 and Section D.1)
- 7. Results of any performance analysis that was performed for the written examination, (No recommended substantive changes to be made) (ES-403)

- 8. Original Form(s) ES-201-3, "Examination Security Agreement," with a pre-and post-examination signature by every individual who had detailed knowledge of any part of the operating tests or written examination before they were administered.
The post examination performance analysis identified potential areas of weakness. Three questions on the Senior Reactor Operator Exam were missed by 50 percent or more of the applicants (SRO-10, SRO-17, & SRO-22). There were no questions missed by 50 percent or more on the RO portion of the Exam. This condition was entered into the Corrective Action Program under Condition Report 2021 -01206 for further analysis.
