St. Lucie Units 1 and 2 Docket Nos. 50-335 and 50-389 Annual Fitness-For-Duty Program Report - Revised On February 24, 2015, in accordance with 10 CFR 26.717, Florida Power & Light Company (FPL) forwarded FPL letter L-2015-050, fitness-for-duty program performance data for 2014. The purpose of this letter is to provide an updated section of the report; 10 CFR Part 26, Subpart I, Annual Fatigue Results. This correspondence supersedes FPL letter L-2015-050 in its entirety.

During a recent FPL internal audit of the Fitness-for-Duty and Access Program, including work hour controls, it was identified that three Security officers were assessed for "Self-Declaration" fatigue assessment, two were determined to be impaired and given 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> off and one Officer returned to work. This was reported in error in that all three officers were determined to be impaired and given 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> off to obtain adequate rest. This error was entered into the St. Lucie corrective action system for evaluation and associated action.

As part of the follow-up actions, attached are the revised forms for the 2014 Annual Fatigue Results. The increased number of fatigue assessments being reported did not have an effect on the performance objectives for work hours controls.

This omission was discussed with Mr. Perry Buckberg - NRR Project Manager, on September 2, 2015.

There were nine waivers performed in 2014. Eight of the waivers were related to an emergent down power to support a recovery of foreign1 material in the Unit 2 B steam generator bowel.The waivered individuals performed activities to support SIG nozzle dam removal, installation of SG manways and tube plugging evaluations to enable exiting RCS midloop conditions. One additional waiver was necessary forea valve repair by a Furmanite Technician with unique qualifications. Analysis of Fatigue Assessment Data:

Eight online fatigue assessments were conducted in 2014: 1) Five workers were assessed "For-Cause," two (2) Security Officers, two (21 Contractor workers, and one(1) Maintenance worker. The Security Officers were determined to be impaired, given 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> offeand advised of strategies for obtaining adequate rest or combating fatigue. The Maintenance worker was determined to be impaired and was given 1 0 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> offeand advised of strategies for obtaining adequate rest or combating fatigue, the two (2) Contractors wore determined not to be impaired and continued to work, 2) Three Security Officers were assessed for "Serf-Declaration" fatigue assessments. The Officers were determined to be impaired, given 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> off and advised of strategies for obtaining adequate rest or countering fatigue.

Conclusions:==

There were a total of nine (9) waivers issued in 2014. The waivers wore not required for safety or security reasons but was required to assure the identifed tasks could be performed to enable exiting mid-Ioop conditions.

There ware no corrective actions taken in response to the use of waivers and the conduct of fatigue assessments in 2014.
