Subject: NOTICE OF VIOLATION AND ENFORCEMENT CONFERENCE MEETING  !

(NRC Inspection No. 50-354/87-17)

This refers to the NRC safety inspection conducted on July 14 - August 17, 1987, ,

at the Hope Creek Generating Station to review the circumstances associated with i violations identified by your staff and reported to the NRC including a vio-lation of a Technical Specification Limiting Condition for Operation. The inspection report was sent to you on October 5, 198 On October 19, 1987, an enforcement conference was conducted with Mr. S. Miltenberger and members  ;

of your staff to discuss the violations, their causes, and your corrective '

actions. A summary of the enforcement conference and your presentation notes are enclosed with this lette The violations, which are described in the enclosed Notice, include a degradation of the permissive logic which allows certain ADS valves to open u the event of an actuation signal. The degradation was caused by a mispositioned instrument root valve associated with the two discharge pressure transmitters for one of the four RHR pumps. As a result, the transmitters would not perform ,

their intended safety function of sending a permissive signal to ADS indicating l that the pump was operating. Such a signal is needed before ADS would actuat The NRC recognizes that the other three RHR pumps were operable, as were both Core Spray subsystems, and if required would have sent the permissive signal to ADS. Therefore, since the safety significance of this violation is small, l

given these multiple redundant safety features, the violation is classified at Severity Level IV in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR 2, Appendix C (Enforcement Policy) (1987). The other violation described in the Notice is also classified at Severity Level I i Nonetheless, the NRC is concerned that this degradation, which was identified on July 11, 1987, may have existed since April 10, 1987 and was not promptly recognized nor were controls established to limit work on the operable redundant component Furthermore, this violation was similar to a violation  !

previously cited in December, 1986 (Reference: Inspection Report 50-354/86-48), )

Therefore, we emphasize that any similar violations in the future may result in additional enforcement actio You are required to respond to the enclosed Notice and, in preparing your response, you should follow the instructions specified in the Notic In your response, you should document the actions taken or planned to prevent recurrence. In particular, you should address the measures you have established to assure that your corrective actions regarding causal analysis and prevention of recurrence have been effective. After reviewing your response to the Notice, including your proposed corrective actions and the results of future inspections, the NRC will determine whether further enforcement action is necessary to ensure compliance with NRC regulatory requirement In accordance with 10 CFR 2.790, a copy of this letter and its enclosures will be placed in the NRC Public Document Roo The responses directed by this letter and the enclosed Notice are not subject to the clearance procedures of the Office of Management and Budget, otherwise required by the Paperwork Reduction Act of 1980, Pub.
