I. DESCRIPTION OF THE REPORTABLE EVENT

A. REPORTABLE EVENT CLASSIFICATION.

This event is* reportable under 1 OCFR50.73(a)(2)(iv)(A), "Any event or condition that resulted in manual or automatic actuation of any of the systems listed in paragraph (a)(2)(iv)(B) of this sec;tion.n The system that actuated was the Unit 2 Auxiliary Feedwater System

  • B. PLANT CONDITION PRIOR TO EVENT At 2151 on May 14, 2019, Comanche Peak Nuclear Power Plant (CPNPP) Unit 2 Wp,S in MOOE 1 operating at 100°/o power.

C. STATUS OF STRUCTURES, SYSTEMS, OR COMPONENTS THAT WERE INOPERABLE AT THE START OF THE EVENT AND THAT CONTRIBUTED TO THE EVENT At the start of this event, the Stephenville 138 KV transmission line tp the CPNPP 138 KV switchyard. tripped.

D. NARRATIVE SUMMARY OF THE EVENT, INCLUDING DATES AND APPROXIMATE TIMES At 2151 on May 14, 2019; CPNPP experienced a loss of the Stephenville 138 KV transmissiqn line [Ell~: (FK)(JX)]. This resulted in a transfer of the Unit 2 Safeguards buses to th.e alternate off site power source and an actuation of both Unit 2 blackout sequencers and an automatic start of both Unit 2 Motor Driven Auxiliary Feedwater pumps (MDAFWPs) [EIIS:

(BA){P)] and the Unit 2Turbin~ Driven Auxiliary Feedwater purnp (TDAFWP) [EIIS: {BA)(P)]. The Emergency Diesel.

Generators did not start since they were not required to start due to a successful bus transfer. The MDAFWPs and

  • TDAFWP were returned to automatic. The off site source was recovered at 0256 on May 15, 2019. There was n.o impact onUnit1; E. THE METHOD OF DISCOVERY OF. EACH COMPONENT OR SYSTEM FAILURE, OR PROCEDURAL PERSONNEL ERROR Operators (Utility, Licensed) in the !.Jnit 2 Control Room received al;:1rms relat~d to "Loss Of Startup/Station SE:lryice Transformers."

IV.. CAUSE OF THE EVENT.

The loss of the St.ephenville 138 KV transmission line was ca.used by a fault in the Stephenville switc~yard that is owned by,another electric compf!ny; V. COR~ECTIVE ACTIONS.

The MDAFWPs and the TDAFWP were:returned to automatic. The off site source was recove.red at 025(:fon May Hi; 2019. There were no component or system failures with any CPNPP components during this event. ttowever the off.
