- 14. Supplemental Report Expected No Yes (If yes, complete 15. Expected Submission Date)
- 15. Expected Submission Date Month Day Year V. C. Summer Nuclear Station (VCSNS) identified vulnerabilities in multiple procedures governing operation of the Feedwater System. On January 21, 2026, review found at least eight instances where these procedure vulnerabilities resulted in non-compliance with Technical Specification (TS) Limiting Condition for Operation (LCO) 3.3.2, Table 3.3-3, Item 6.g. During implementation of required corrective actions, on February 7, 2026, an operational mode change occurred without meeting the TS LCO 3.3.2, Table 3.3-3, Item 6.g.
This TS requirement specifies that with less than the required channels for Trip of Main Feedwater Pumps Start Motor-Driven Pumps, the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
. Contrary to that TS requirement, multiple procedures did not contain adequate guidance to ensure timely placement of the inoperable channel into the tripped condition.
Per 10 CFR 50.73(a)(1), this report under 10 CFR 50.73(a)(2)(i)(B), includes past operations prohibited by Technical Specifications within the prior three years, in addition to the February 7, 2026 event.

3.0 CAUSE OF THE EVENT
Past non-compliances were the result of inadequate procedural guidance to ensure placement of inoperable channels in the tripped condition within 1 hour1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
. The RCE for the February 7, 2026 event determined there were inadequate procedural and programmatic controls that did not ensure TS 3.3.2, Table 3.3-3, Item 6.g compliance. Additionally, interim Standing Order SO 26-001 did not contain sufficient level of detail to ensure the crews complied.
4.0 CORRECTIVE ACTIONS
On January 22, 2026, Operations issued Standing Order SO-26-001 to provide interim guidance to support compliance with the TS requirement, while updates to the vulnerable procedures were processed. On February 7, 2026, an additional non-compliance occurred, indicating inadequate interim actions. Additional interim actions were implemented to address contributors to this latest event and the SO was revised.
Corrective action to preclude repetition (CAPRs) were established during the RCE. The intent of the CAPR is to ensure effective procedural and programmatic controls are in place.
5.0 PREVIOUS OCCURENCES
This LER documents multiple instances of TS non-compliance caused by a common cause (inadequate guidance in operating procedures). Review of other recent LERs under this reporting requirement for TS non-compliance did not identify TS non-compliance caused by procedure deficiency.
