Abstract
V.C Summer Nuclear Station, Unit 1 395 3
OPERATIONS PROHIBITED BY TECHNICAL SPECIFICATIONS 01 21 2026 2026 001 00 03 20 2026 1
100 William Kirkland, Staff Engineer 803-941-9232 05 15 2026 VC Summer Nuclear Station (VCSNS) identified vulnerabilities in multiple procedures governing operation of the Feedwater System. On January 21, 2026, review found at least eight instances where these procedure vulnerabilities resulted in non-compliance with Technical Specification (TS) Limiting Condition for Operation 3.3.2, Table 3.3-3, Item 6.g. During implementation of required corrective actions, on February 7 an operational mode change occurred without meeting the Technical Specification (TS) Limiting Condition for Operation 3.3.2, Table 3.3-3, Item 6.G.
This TS requirement specifies that with less than the required channels for Trip of Main Feedwater Pumps Start Motor-Driven Pumps, the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days
2.777778e-4 hours
1.653439e-6 weeks
3.805e-7 months
. Contrary to that TS requirement, multiple procedures did not contain adequate guidance to ensure timely placement of the inoperable channel into the tripped condition.
Per 10 CFR 50.73(a)(1), this report under 10 CFR 50.73(a)(2)(i)(B), includes past operations prohibited by Technical Specifications within the prior three years, in addition to the February 7, 2026 event.Page of

4.0 CORRECTIVE ACTIONS
On January 22, 2026, Operations issued Standing Order SO-26-001 to provide interim guidance to support compliance with the TS requirement, while updates to the vulnerable procedures were processed. On February 7, an additional noncompliance occurred, indicating inadequate interim actions. Additional interim actions were implemented to address contributors to this latest event. The Root Cause Evaluation into this recurrent noncompliance will determine the cause and additional corrective actions, which will be communicated in a future supplement to this LER.
5.0 PREVIOUS OCCURENCES
This LER documents multiple instances of TS noncompliance caused by a common cause (inadequate guidance in operating procedures). Review of other recent LERs under this reporting requirement for TS noncompliance did not identify TS noncompliance caused by procedure deficiency.
