Identification of Occurrence:

Technical. Specification 4.7.4.1. requires each service water valve servicing safety-related systems to be either locked open or have its

. position verified every 31. days.- During a Quality Assurance audit which was performed on October 7,1982 the. service water supply valves to the safety injection (SI) and centrifugal charging pump (CCP) lube oil coolers were found not to be on the locked valve list. Further investigation identified these same valves as not having locking de-vices nor having their position verified every 31 days.

Conditions Prior to Occurrence:

The Plant was operating in Mode 1 at 100% power when the deficiency-was discovered.

Description of Occurrence:

The eight service water valves (pump skid mounted) to the SI pump and CCP lube oil coolers are required to be controlled as locked open or the valve position verified every 31 days in accordance with Trojan Technical Specification 4.7.4.1.

During a routine audit it was found that in addition to isolation valves for each service line to'the.

safety injection and centrifugal charging pumps, there are also indivi-dual isolation valves on each pump skid which are not shown on Trojan piping and instrument diagrams. As such, the valves were not on the locked valve list (AO-3-13) nor were they provided with locking devices, and the valve positions were not verified every-31 days.

Designation of Apparent Cause of Occurrence:

The service water valves supplying the safety injection and charging pump lube oil coolers were not shown on the piping'and instrument diagrams (P& ids) which were used to determine valves to be included in the locked valve' list. As the valves were not shown in the'P&ID, they were not included in the periodic verification of valve position re-quired by the Technical Specifications.

Significance of occurrence:

This occurrence had no effect on plant or public safety. The valves were already open and they were immediately tagged to remain in the open position. They have now been added to the locked valve list to ensure adequate control'in accordance with the~ Trojan Technical Specs.

Corrective Ar ilon:

Immediate corrective action was taken to perform the required surveil-lance testing. The service water valves.were tagged in the open position and added to 'the -locked valve ' list. ' Subsequent corrective action taken was to. install locking devices on the valves. A survey-of-other skid mounted safety-related pumps will be completed by December 31, 1992 to ' determine ~if other similar discrepancies exist.
