In accordance with the Trojan Plant Operating License, Appendix A, US NRC Technical Specification 6.9.1.9.b, attached is Licensee Event Report No.

81-28, concerning a situation where the room cooling fan for the "B" Train Safety Injection Pump failed to auto start during periodic testing. When subsequent manual actuation failed to start the pump, the "B" Train Safety Injection Pump was declared inoperable.

Identification of Occurrence:

During periodic testing the "B" train safety injection (SI) pump room cooler failed to auto start. When the cooler would not subsequently start manually the~ associated pump was declared inoperable.

Conditions Prior to Occurrence:

The plant was in Mode 1 at 100% power, steady state operation.

Description of Occurrence:

The periodic operating test for the SI pumps requires the pump room cooler to auto start following a start signal to the SI pump. When the fan failed to auto start and subsequently failed to manually start the pump was declared inoperable due to potentially inadequate cooling to the room.

Therefore the plant was operating in a degraded mode permitted by the Technical Specifications, paragraph 3.5.2.

Designation of Apparent Cause of Occurrence:

The cause of the failure was discovered to be a failure of the room cooler thermal overload relay to mechanically latch.

8.

Significance of Occurrence:

There was no effect on either plant or public safety since the redun-dant SI train was operable and no conditions requiring the safety injection pumps existed during the time the cooler was out of service.

9.

Corrective Action

The corrective action taken was to replace the thermal overload relay mechanism.
