in accordance with Technical Specification Appendix A,"$ection 6.9.4.2.b,

" Operation in a degraded mode permit ted by a 1imi ting condi tion for operation", and Section 3.12, " Shock Suppressors", the Sacramento Municipal Utility District hereby submits a thi rty-day report of Reportable Occurrence 81-46.

On September 10, 1081, wi th the reactor shutdown for turbine repai rs, performance of the surveillance inspection for snubbers revealed a non-functional snubber.

Continued performance of the surveillance over the next two weeks revealed five additional snubbers that were low on fluid. The first snubber was foui.) unrestrained; the clevis pin was missing; and three of four pipe line clamp nuts were missing.

It was properly reinstalled, with both ends restrained.

It was inspected and found to have suf ficient hydraulic fluid. The pipe clamp nuts were installed. This was a Bergan Patterson 10 kip, 6" stroke snubber on the pressurizer surge line.

The second snubber was found l oi, in fluid.

It was removed, tested and found to be functional.

It was refilled and reinstalled.

The third snubber was found low in fluid.

It was removed, tested, and found to be non-functional. The valve block body was rebuilt, and the snubber was refilled and reinstalled.

This was a Grinnell 3 kip, 1.5" size snubber on a reactor coolant pump seal supply line.

The fourth snubber was found low in fluid. Since it is a super snubber, it was assumed to be non-functional.

It was removed, the valve block assembly was rebuilt, and the seals replaced. It was refilled and replaced. This was a Bergan Patterson 70 kip, 6" stroke snubber on reactor coolant pump "C" QRADOCK10160347 311005 hI l 05000312 1

It was removed, tested, and found to be non-functional.

It was rebuilt, refilled, and reinstalled.

This was a Grinnell 3 kip, 1.5" size snubber on a reactor coolant pump seal return line.

The sixth snubber was found low in fluid, but passed its functional test.

It was refilled and reinstalled.

Because four of the six snubbers were determined or assumed to be non-functional, the surveillance interval will be changed to four months in accc rdance with Technical Specifications.

The reactor remained shutdown, unaffected by these events.
