No. 22 Accumulator Below Minimum Pressure.

This report was initiated by Incident Report 82-201.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 81% - Unit Load 860 MWe.

DESCRIPTION OF OCCURRENCE:

On July 29, 1982, the nitrogen pressure in No. 22 Accumulator dropped below the minimum technical specification pressure of 595.5 psig.

At 0740 hours0.00856 days <br />0.206 hours <br />0.00122 weeks <br />2.8157e-4 months <br />, No. 22 Accumulator was declared inoperable and Action Statement 3.5.1.d was entered.

This occurrence constituted operation in a degraded mode in accordance with-Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause was operator error.

ANALYSIS OF OCCURRENCE:

i The accumulators are designed to automatically inject borated water into the primary system to rapidly cool the core in the event of a system rupture.

Nitrogen pressure below normal would reduce the effectiveness of an accumulator.

However, four accumulators in the system provide the proper redundancy to protect against that possibility.

At no time was the safety of the environment and general public in jeopardy.

CORRECTIVE ACTION

The accumulator nitrogen pressure was immediately restored to above 595.5 psig.

At 0745 hours0.00862 days <br />0.207 hours <br />0.00123 weeks <br />2.834725e-4 months <br />, No. 22 Accumulator was declared operable and Action Statement 3.5.1.d was terminated.

The control operator was counseled on the proper control of accumulator nitrogen pressure.
