No. 21 Containment Spray Pump - Inoperable.

This report was initiated by Incident Report 81-494.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 70% - Unit Load 790 MWe DESCRIPTION OF OCCURRENCE:

On December 9, 1981 the No. 21 Containment Spray Pump 125 Volt D.C.

alarm was produced on the Auxiliary Alarm Typewriter.

It was verified that the 125 Volt D.C.

power switch was in the ON position.

The control operator attempted to start No. 21 Containment Spray Pump from the control room, unsuccessfully.

At 0400 hours0.00463 days <br />0.111 hours <br />6.613757e-4 weeks <br />1.522e-4 months <br />, No. 21 Containment Spray Pump was declared inoperable and Action Statement 3.6.2.1 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Investigation revealed that internal component failure in the 125 Volt D.C.

breaker caused the contacts to open without tripping the external switch.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.1 requires:

CORRECTIVE ACTION

The 125 Volt D.C. breaker was properly replaced and tested satisfac-torily.

At 1109 hours0.0128 days <br />0.308 hours <br />0.00183 weeks <br />4.219745e-4 months <br />, No. 21 Containment Spray Pump was declared operable and Action Statement 3.6.2.1 was terminated.

During the time No. 21 Containment Spray Pump was inoperable, the plant was adequately protected by Nc. 22 Containment Spray Pump.
