2A Safeguards Equipment Control - Inoperable.

This report was initiated by Incident Report 81-400.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3'- Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On October 6,.1981, while performing the modifications specified in' Design Change Package (DCP) 2-EC-ll50, a Vitro Labs repre-sentative accidently allowed some: solder to fall into the 15 volt power supply for the 2A Safeguards Equipment Control'(SEC),

causing the loss of the. power supply._ At 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> 2A SEC was declared inoperable and Action Statement 3.3.2 Action 13 was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel error.

The solder fell into the power supply due to a lack of proper protective covering.

ANALYSIS OF OCCURRENCE:

1 Technical Specification 3.3.2 Action 13 requires:
