Pressurizer Pressure Channel 1 - Inoperable This report was initiated by Incident Report 81-347.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1120 MWe DESCRIPTION OF OCCURRENCE:

On August 28, 1981, it was discovered that Pressurizer Pressure Protection Channel 1 was in the tripped condition, accompanied by a low pressurizer pressure, one out of three, annunciator alarm.

However, the Channel 1 indicator provided proper indication, in agreement with the other two channels.

This indicated-that the malfunction existed in.the protection channel itself.

At 0315 hours0.00365 days <br />0.0875 hours <br />5.208333e-4 weeks <br />1.198575e-4 months <br />, August 28, 1981, action statements 3.3.1, action 6 and 3.3.2, action 14 were entered.

This occurrence constituted operation in a degraded mode in accord-ance with tech. cal specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Component failure.

An A417 universal board in the Solid State Protection System, Train A, had failed.

l ANALYSIS OF OCCURRENCE:

Technical specification 3.3.1, action 6 requires:

With the number of operable channels one less than the total number of channels, startup and/or power operation may proceed provided the inoperable channel is placed in the tripped condition.

CORRECTIVE ACTION

Pressurizer Pressure Channel 1 was placed in the tripped condition within one hour.

Universal board A417 in the Solid State Protection System, Train A, was replaced.

The channel was tested satisfactorily and returned to service.
