December 15,1982 IDENTIFICATION OF OCCURRENCE:

The Control Complex Ventilation Radiation Monitor (RMA-5) required by Technical Specification 3.3.3.1 was inoperable.

CONDITIONS PRIOR TO OCCURRENCE:

MODE 5 (COLD SHUTDOWN)

DESCRIPTION OF OCCURRENCE:

At 1825 on December 15, 1982, it was discovered that the circuit breaker on RMA-5 tripped resulting in a loss of power to the monitor.

DESIGNATION OF APPARENT CAUSE:

This event occurred when the RMA-5 pump vane broke causing the pump to bind and subsequently tripping the supply breaker.

ANALYSIS OF OCCURRENCE:

The function of RMA-5 is to switch the Control Room Ventilation System to a Recirculation Mode upon high radiation signal. Therefore, the inoperability of this monitor does not impact off-site release (s) or the monitoring thereof, and thus has no direct impact on public health or safety.

CORRECTIVE ACTION

The Control Complex Ventilation System was placed in recirculation and the pump was replaced.

The pump was tested satisfactorily and operability was restored at 2115 on December 15,1982.

FAILURE DATA:

This is the first occurrence of this type for RMA-5 and the nineth event reported under this specification.
