SUPPLEMENTARY INFORMATION Report No.:

January 6, 1982 Occurrence Date: December 8, 1981.

Identification of Occurrence:

Emergency Feedwater Ultrasonic Flowrate Indicator was inoperable contrary to Technical Specification 3.7.1.2.

Conditions Prior to Occurrence:

Mode 3 (0%).

Description of Occurrence:

At 1310 during normal operation, it was discovered that Emergency.

Feedwater Flow Indicator, FW-313-F1, exceeded the zero check low limit. Maintenance was initiated, and operability was restored at 1700 on December 9, 1981.

Designation of Apparent.Cause:

The cause of this event is attributed to inherent inability of the instrument to consistently indicate zero flow.

Analysis of Occurrence:

Redundancy was provided by Ultrasonic Flow Indicator FW-312-F1.

There was no effect upon the health or safety of the general public.

Corrective Action

The instrument was re-zerced and is checked shiftly, as reported in Licensee Event Report 81-048. The Ultrasonic Flow Transmitters are scheduled to be' replaced with conventional flow transmitters in conjunction with major feedwater modifications currently scheduled for 1983.

Failure Data:

This was the second occurrence of this type, and this is the eleventh event reported under this Specification.
