Quadrant Power Tilt Ratio - Out-of-Epecification.

This report was initiated by Incident Report 82-254.

7 CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1110 MWe.

I DESCRIPTION OF OCCURRENCE:

At 0230 hours0.00266 days <br />0.0639 hours <br />3.80291e-4 weeks <br />8.7515e-5 months <br />, September 6, 1982, calculation showed the Quadrant Power Tilt Ratio (QPTR) for No. N42 Bottom Detector I

was greater than the maximum of 1.02 allowed by the Technical c

Specifications.

Since the value was within the range of 1.02

to 1.09, Technical Specification Action Statement 3.2.4.a was entered.

Investigation into the cause of the excessive QPTR revealed that a problem existed in No. N42 Detector B drawer.

A current measurement was taken directly from the detector, i-and the OPTR was determined to be within specification.

Action Statement 3.2.4 was accordingly terminated at 0425 hours0.00492 days <br />0.118 hours <br />7.027116e-4 weeks <br />1.617125e-4 months <br />, September 6, 1982.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Further investigation of the problem revealed that the milli-amp detector switch in the detector current meter assembly had failed.

The failure resulted in improper detector current E

measurements and an erroneous QPTR value.

No previous failures of the switch had been noted, and the occurrence was assumed to k

be of an isolated nature.

ANALYSIS OF OCCURRENCE:

The limit on QPTR provides DNB and linear heat generation rate 7

protection with X-Y plane power tilts.

Action Statement 3.2.4.a i

, ANALYSIS OF OCCURRENCE:The 2 hour2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> time allowance for operation with QPTR between 1.02 and 1.09 is provided to allow identification and correction of a dropped or misaligned rod.

As noted, the QPTR was actually within specification.

The occurrence, therefore, did not involve any risk to the health or safety of the public.

The incident constituted operation in a degraded mode permitted by a limiting condition for operation, and is reportable in accordance with Technical Specification 6.9.1.9.b.

CORRECTIVE ACTION

As stated, the QPTR was determined to be within specification, and the action statement was terminated within the 2 hour2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> time limit.

The failed assembly was replaced, the channel was satis-factorily tested and was returned to service.

In view of the nature of the failure, no further action was deemed necessary.
