2C Vital' Bus Undervoltage Relay - Inoperable.

This report was initiated by Incident Report 81-476.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -1Rx Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

On November 23, 1981, during the performance of the M3T maintenance surveillance test, it.was discovered that the_undervoltage relay on the 2C vital bus was actuating at 35% instead of the'70% trip-setpoint value.

The minimum required trip setpoint value is 65%.

The relay was placed in-the tripped condition, and at 1557 hours0.018 days <br />0.433 hours <br />0.00257 weeks <br />5.924385e-4 months <br /> Action Statement 3.3.2.a Action 14 was entered.

This occurrence constituted operation in a degraded mode in accord-ance with Technical Specification 6.9.1.9.a.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The undervoltage relay actuated at 35% instead of the 70% trip setpoint value-because it was mechanically sticking.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2.a Action 14 requires:

With the number of operable channels one less than the total number of channels, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION

The undervoltage relay was cleaned and tested satisfactorily.

At 0915 hours0.0106 days <br />0.254 hours <br />0.00151 weeks <br />3.481575e-4 months <br />,' November 24, 1981, Action Statement-3.3.2.a Action 14 was-terminated. 'This is the first occurrence.of its. type on this unit.
