Fire Detection Instrumentation - Inoperable.

This report was initiated by Incident Report 81-515.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 62% - Unit Load 640 Mwe DESCRIPTION OF OCCURRENCE:

On December 29, 1981, during the performance of the surveillance test for wet pipe sprinkler flow for the Service Building, 88' Elevation, the audible alarm emitted only a continuous tone, without the coded signal.

At 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br /> breaker No. 13 in the Miscellaneous Signal. Cabinet was deenergized, rendering the alarm inoperable, and Action Statement 3.3.3.6 was entered.

An operator was stationed at the 1RP5, Fire Zone Alarm panel, to track alarm status.

.This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The coded signal did not sound because the encoder was mechanically jammed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.6 requires:

With the number of operable fire detection instruments less than required by Table 3.3-10:

Within one hour establish a fire watch patrol to inspect the zone (s) with the inoperable instrument (s) at least once per hour, and restore the inoperable instrument (s) to operable status within 14 days, or in lieu of any other report required by specification 6.9.1, prepare and submit a special report to the commission pursuant to specification 6.9.2 within the next 30 days.

The mechanical mechanism in the encoder was unjammed and tested satisfactorily.
