IDENTIFICATION OF OCCURRENCE:

No. 12 Charging Pump - Inoperable.

This report was initiated by Incident RepUrt 81-489.

CONDITIONS PRIOR TO OCCURRENCE:

i Mode 1 - Rx Power 75% - Unit Load 800 MWe DESCRIPTION OF OCCURRENCE:

I On December 6, 1981 the shift supervisor discovered a service water leak on No. 12 Charging Pump on the return line of the oil cooler.

It was isolated by closing 1SW912 and 1SW916, the service water in-let and outlet valves to the lube oil cooler, pump gland cooler, and gear oil cooler, rendering-the charging pump inoperable.

At 1134 hours0.0131 days <br />0.315 hours <br />0.00187 weeks <br />4.31487e-4 months <br />, No. 12 Charging Pump was declared inoperable and Action Statement 3.5.2.a was entered.

l This occitrrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The leak was caused by corrosion within the pipes.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.5.2.a requires:

With one ECCS subsystem inoperable, restore the inoperable subsystem to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION

The leak was repaired by replacing the carbon steel pipe tee in the return line of the gear oil cooler to the pump gland cooler.

No. 12 Charging Pump was tested satisfactorily, and at 1907 hours0.0221 days <br />0.53 hours <br />0.00315 weeks <br />7.256135e-4 months <br />, December 6, 1981, the pump was declared operable and Action State-ment 3.5.2.a was terminated.

The plant was adequately protected by the other two Charging Pumps.

A request has been submitted for a DCR to upgrade the service water piping on the Charging Pumps.
