Containment Fan Coil Unit - Inoperable.

This report was initiated by Incident Report 81-450.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On November 6, 1981, during recovery actions from an inadvertent safety injection, the No. 11 Containment Fan Coil Unit (CFCU) failed to start in low speed mode in response to a safety injection signal.

The No. 11 CFCU was declared inoperable and Action Statement 3.6.2.3.a was entered at 1411 hours0.0163 days <br />0.392 hours <br />0.00233 weeks <br />5.368855e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The reason the No. 11 CFCU did not start was that the automatic trip shutter on the low speed circuit breaker had failed to close when the breaker was racked in by the operator causing the breaker to trip every time it was reset.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.2.3.a requires:

With one group of the required containment cooling I

CORRECTIVE ACTION

The automatic trip shutter on the No. 11 CFCU circuit breaker was manually closed and the breaker was tested satisfactorily.

Action Statement 3.6.2.3.a was terminated at 2110 hours0.0244 days <br />0.586 hours <br />0.00349 weeks <br />8.02855e-4 months <br /> on November 6, 1981.

Equipment operators will be advised of correct procedures for verification of breaker operability.
