Violation of Amendment 39 to Technical Specification 3.4.3.a 1

(Inoperable Power Operated Relief Valve).

This report was initiated by Incident Report 81-419.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 97% - Unit Load 1060 MWe.

DESCRIPTION OF OCCURRENCE:

At 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br /> on October 21, 1981, the Senior Shift Supervisor was notified by the NRC' Inspector that Unit 1 was in violation of the requirements of Amendment 39 to Technical Specification 3.4.3.a which requires that "when one or more Power Operated Relief Valves (PORV) are inoperable then all associated block valves shall be closed and power removed from the valves".

Action Statement 3.4.3.a was entered at 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br />.

This occurrence constituted operation in a degraded mode i

in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

i At the time of the incident, this station was not in possession of a copy of Amendment 39 to the Technical Specification and therefore was not aware of the violation.

ANALYSIS OF OCCURRENCE:

Amendment 39 to Technical Specification 3.4.3.a states:

CORRECTIVE ACTION

The Senior Shift Supervisor removed the power from-the block valves which had previously been closed and posted Amendment 39.in the Technical Specifications book.

This station will be in full compliance with Amendment 39 of Technical Specifications of Facility Operating License DPR-70 upon return from Unit I refueling outage.
