Pressurizer Level Channel 3 - Inoperable.

This report was initiated by Incident Reports81-392 and 81-394.

CONDITIONS PRIOR TO OCCURRENCE:

9-29-81 Mode 1 - Rx Power 97% - Unit Load 1030 MWe 10-1-81 Mode 1 - Rx Power 98% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On two separate occasions, September 29, 1981 and October 1, 1981, an operator noticed that the Channel 3 Pressurizer Level was reading higher than the other two channels.

At 0720 and 0835 hours0.00966 days <br />0.232 hours <br />0.00138 weeks <br />3.177175e-4 months <br />, respectively, the channel was declared inoperable, and limiting condition for operation (LCO) 3.3.1.1, Action 7 was entered.

These occurrences constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The Channel 3 transmitter had drifted out of calibration.

ANALYSIS OF OCCURRENCE:

LCO 3.3.1.1 Action 7 requires:

With the number of operable channels one less than the total number of channels, start-up and/or power operation may proceed until performance of the next required channel functional test provided the in-operable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

CORRECTIVE ACTION

On both occasions the Channel 3 Pressurizer Level bistable was placed in the tripped condition within one hour.

On September 29 the transmitter was recalibrated, tested satisfactorily, and returned to service.

On October 1 the transmitter was backfilled to renove trapped air, recalibrated, tested satisfactorily, and returned to service.
