No. 13 Containment Fan Coil Unit - Inoperable.

This report was initiated by Incident Report 81-341.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 96% - Unit Load 1040 MWe PP3CRIPTION OF OCCURRENCE:

g On August 24, 1981, the control operator noticed that No. 13 containment fan coil unit (CFCU) indicated no service water flow in high speed.

Investigation revealed a substantial leak on 13SW72, a valve upstream of the flow transmitter.

The leak was in the penetration area, and was isolated.

At 2215 hours0.0256 days <br />0.615 hours <br />0.00366 weeks <br />8.428075e-4 months <br />, August 24, 1981, No. 13 CFCU was declared inoperable and action statement 3.6.2.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The leak was emanating from a hole in the weld on the pipe flange on 13SW72.

ANALYSIS OF OCCURRENCE:

Action statement 3.6.2.3.a requires:

With one group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inop-erable group of cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The pipe flange was rewelded and tested satisfactorily.

No. 13 CFCU was returned to service, and at 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />, August 30,.1981, action statement 3.6.2.3.a was terminated.
