Salem Generating Station, Unit 1 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

Containment Fan Coil Unit (CFCU) - Inoperable - No Service Water Flow.

This report was initiated by Incident Report 81-264.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 96% - Unit Load 1010 MWe DESCRIPTION OF OCCURRENCE:

On July 20, 1981, while attempting to place the No. 15 CFCU in in service the control room operator noticed that there was no service water flow at high speed and insufficient flow at low speed.

The CFCU was declared inoperable and action statement 3.6.2.3.a was entered at 1811 hours0.021 days <br />0.503 hours <br />0.00299 weeks <br />6.890855e-4 months <br />, July 20, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with cochnical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The high pressure side of flow transmitter FA-3543Z-1 was plugged solid with silt.

ANALYSIS OF OCCURRENCE:

Technical specification 3.6.2.3.a requires with:

One group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inoperable group of cooling fans to operable status within 7 days or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

Flow ifansmitter FA-3543Z-1 sensing line was blown clear and the transt

ter was recalibrated satisfactorily.

The No. 15 CFCU was tected satisfactory attd returned to service.

Action statement 3.6.2. 3. a was tenninated at 0050 hours5.787037e-4 days <br />0.0139 hours <br />8.267196e-5 weeks <br />1.9025e-5 months <br /> on July 21, 1981.

A program is currently in effect to blow down all transmitters at regular intervals.
