lB Diesel - Inoperable'- Water' Jacket Leak

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load OMW DESCRIPTION OF OCCURRENCE:-

' During routine' operation, an operator making rounds discovered the 1B Diesel Water Jacket was. leaking via a 1/4" pipe nipple.- Action ~

Statement'3.8.1.1 was entered at 05045 hours on May'20, 1981..

This occurrence' constituted operation in a degraded mode in accordance-

'with Technical Specification 6.9.1.9b.

DESIGNATION OF' APPARENT CAUSE OF OCCURRENCE:

- Personnel error.

This occurrence was caused by apparent use of the

' nipple-for-support'by an unknown person.

i ANALYSIS OF OCCURRENCE:.

.
